The NHS and local partners are working together on new plans to make health and care services easier, faster and more joined up for everyone in our communities.
We want to bring more care closer to where you live, so that:
✅ You can get help earlier
✅ Services work together – no more repeating your story
✅ Support is available locally, not only in hospital
✅ Health, care, council and charity teams can work as one team around you
But before we make any changes, we want to hear from you.
Do these ideas feel helpful? Would they improve your experience of using health or care services? What really needs to change to make things work better for you, your family and your community?
Your voice will directly shape our Neighbourhood Health plans.
Please take a few minutes to fill in our short survey and share your views.
